Digital Equity Survey

New York State has recognized the importance of access to reliable internet (broadband) and devices for educational purposes. The state education department has developed a survey that each district must distribute to every student to help evaluate where there may be inequities in broadband or device access.

Southwestern will be collecting the survey information with a paper survey that will be sent home to families. At the elementary school, the surveys will be in your child’s VIP folder. Please complete the form for each child in your family and return to the teacher in the VIP folder. At the middle and high school, we will be sending the survey home via mail; your child can turn in the completed forms to the main office.

We plan to send surveys home the second week in January, and we ask you to complete and return the surveys to school as soon as possible. If you need assistance or have questions, please contact Molly Moore at or 716-484-1136, option 3.
